The ICO (International Council of Ophthalmology) exams are touted as the "only worldwide medical-specialty examinations...[that] are independent and free of any outside influence and can be taken in the candidate's own country."  The ICO Standard exams are divided into 3 sections:

  • Visual Sciences.
  • Optics & Refraction and Instruments
  • Clinical Ophthalmology


The Clinical Ophthalmology exam can only be taken after passing the first two Standard exams (or an exam of a similar standard recognized by the ICO). 

Visual Sciences exam (Part A)

This exam is usually taken early in residency training.  Here are some basic facts about the Visual Sciences exam:

  • 120 multiple-choice questions (single best answer out of 4 choices).
  • Only credit given for correct answers (no credit for incorrect choices or those left blank).
  • 3 hour duration. 
  • Results are mailed to candidates in June.
  • The successful candidate will be given a signed certificate indicating whether (s)he has:  (1) Passed with Distinction (i.e. highest tier); (2) Passed with Merit (i.e. middle tier); (3) Passed Visual Sciences (i.e. passed but at lowest tier).

The topics covered are:

  • Anatomy of the Eye, the Orbit and related structures.
  • Embryology.
  • Neuro-Anatomy.
  • Vision, Ocular Physiology, Biochemistry and Cell Biology.
  • Pharmacology.
  • Epidemiology and Statistics.
  • Genetics.
  • Pathology and Micro-biology.

ICO candidates have used the base subscription of OphthoQuestions to prepare and successfully pass their Visual Sciences exam.  Specifically, the Fundamentals section of the base subscription contains these Visual Sciences' topics.

Optics, Refraction, and Instruments (Part B)


This exam is also usually taken early in residency training.  Some basic facts:

  • 60 multiple-choice questions (single best answer out of 4 choices).
  • Only credit given for correct answers (no credit for incorrect choices or those left blank).
  • 90 minutes duration.
  • Results are mailed to candidates in June.
  • The successful candidate will be given a signed certificate indicating whether (s)he has:  (1) Passed with Distinction (i.e. highest tier); (2) Passed with Merit (i.e. middle tier); (3) Passed (i.e. passed but at lowest tier).

The topics covered are:

  • Optics.
  • Refraction and Instruments.
  • Instruments used in Ophthalmology

Clinical Ophthalmology (Part C)

This exam is taken later in training and is only offered after the candidate has passed the Part A and B exams (or a similar standard recognized by the ICO).  Some basic facts:

  • 200 multiple-choice questions (single best answer out of 4 choices).
  • Only credit given for correct answers (no credit for incorrect choices or those left blank).
  • 4 hour duration.
  • Results are mailed to candidates in June.
  • The successful candidate will be given a signed certificate indicating whether (s)he has:  (1) Passed with Distinction (i.e. highest tier); (2) Passed with Merit (i.e. middle tier); (3) Passed the Clinical Ophthalmology (i.e. passed but at lowest tier).

 The topics covered are:

  • General Medicine related to Ophthalmology.
  • Community and Public Health.
  • Ophthalmic Pathology and Tumours.
  • Neuro-ophthalmology.
  • Paediatric Ophthalmology and Strabismus.
  • Orbit, Eyelids and Lacrimal Disease.
  • External Diseases and Cornea.
  • Intraocular Inflammation and Uveitis.
  • Glaucoma.
  • Lens and Cataract.
  • Retina and Vitreous.
